Our Annual Reports
Following the federation of NHS Aylesbury Vale and NHS Chiltern CCG, the first joint two year Operational Plan has been written. The 2017/19 Operational Plan sets out what the two CCGs, working as one team, will deliver as part of the wider local Buckinghamshire system and the Buckinghamshire, Oxfordshire and Berkshire West Sustainability and Transformation footprint.

Communication and Engagement Plans
We work through our membership, to improve the health of the local population and the quality of the health services they receive. In order to do this most effectively, AVCCG works in partnership with a number of other local organisations, including NHS Chiltern Clinical Commissioning Group, Buckinghamshire County Council and provider organisations. This partnership approach also applies to the way in which communications and engagement is delivered at a county-wide level

Better Care Fund
The purpose of the Outline Business Case is to build on the work done in developing Buckinghamshire’s Better Care Fund submission, and the Case for Change which was developed in February 2014. The Outline Business Case articulates a new integrated system of health and social care for Buckinghamshire and provides a framework for discussion and further consideration, and developement of how this model will be delivered in the next phase of work to develop a strategic Business Case. The wise programme of transformational activity which has been described takes account of existing initiatives that aim to optimise current service delivery and out in place the foundations for new model of care.

Operational Development Plan
The Organisational Development Plan describes our continuing focus on developing distributive clinical leadership and robust management support to enable us to be as fit for purpose as possible in meeting the needs of our patients and populations.
Primary Care Plan
Our Primary Care Strategy for proactive, co-ordinated, out-of-hospital care has been developed in the last 12 months and sets out our vision for community based care. It describes six strategic goals and gives examples of how these may be delivered in the future.
